Before Your Appointment

We ask new patients to: • Arrive 15 minutes earlier than their scheduled appointment to fill out new patient paperwork. • Bring a drivers licence and insurance card. • Bring a list of current medications.

During Your Visit

Expect to fill out new patient paperwork which includes patient information, medical data, medication list, general consent treatment, insurance agreement, record release authorization/assignment of benefits, office policies, and HIPAA forms. A consultation involves a thorough one on one review of your medical history, medications, and discussions with a specialist. From there, the specialist will explain your diagnosis, recommend a course of action, and elaborate on when it should take place. The specialist will also explain treatment options, potential side effects, and further tests or specialist referrals if deemed necessary.
